Acorn nomination  The Southeastern Public Library System of Oklahoma presents its “Acorn” award to employees to recognize them for exceptional service or significant accomplishments, not necessarily on a large scale, which demonstrate our goal of excellence. If you feel one of our employees has shown excellence deserving special recognition, you are invited to nominate the employee. Please give a short written account of how you saw this employee demonstrate excellence in library service or accomplishments.

Library Card Application

 

Directions:

• Complete and print the application.
• Bring the application, a photo ID and proof of mailing address to the library.
• You must be a current resident of or pay property taxes to Pittsburg, Coal, Choctaw, LeFlore, Latimer, Haskell or McCurtain counties to receive a card free for the first time.
• If you are not a resident or do not pay property taxes to one of the above counties, a $25 non-resident fee per family per year will be applied.
• AGE: There is no minimum age for obtaining a borrower’s card,
  However, minors under age 13 must have their registration form
  signed by their parent or legal guardian.  Minors age 13
  through 17 may apply for and be issued a card without the signature
  of a parent or legal guardian.  The library will then mail a
  notice to the parent or legal guardian informing them that the minor
  has been issued a card, with instructions to notify the library if
  they object to issuance of the car.  Otherwise the card will
  remain valid.
